The most current checklists are available for download by clicking below.  We recommend that before you put these into place in your operating rooms that you form a multi-disciplinary team, thoroughly review the checklist content, and modify them as needed.

Customizing the Crisis Checklists at Brigham and Women’s Hospital


Brigham and Women’s Hospital has committed to putting the checklists into place in their operating rooms.  They are in the initial phases of this work and have built a multi-disciplinary implementation team to review and modify the checklists to meet their needs. Over the next several months we will be working together to put the checklists into use in their operating rooms.  

These pictures are the checklists that Brigham and Women’s Hospital modified.  These checklists are posted as examples and are not intended for use. We recommend that every hospital builds an implementation team and modifies the checklists for their needs.
